FinalForms Registration is Now Open for 2026-27

FinalForms registration for the 2026-27 school year is now open to Midview Local School District families who will have student-athletes entering grades 7-12. [FinalForms Site]

To register for athletics for next school year, simply login into FinalForms and click the big blue registration button at the top of your login page and follow all directions.

All forms must be completed (high school students need to sign off on completed forms) and an up-to-date physical must be on file in addition to being on a FinalForms roster for that sport to participate in any official athletic activity. We are asking families to begin uploading their student-athletes physicals straight to FinalForms as PDFs rather than just turning in hard copies this school year. However, hard copies will still be accepted.

For those that are currently training during the off-season, we encourage you to complete this process as soon as you can so that our coaches have the most accurate rosters and emergency contacts heading into the rest of the summer.
